For all those times you need to reach out to someone that cares about the success of East Atlanta, you now have a streamlined place to do that. The form will ask for a case number from 311, so please make sure you've reached out there first (if appropriate). Areas of concern on the District 5 Help Form include the following:
- Applying for Permit/License/Zoning
- Public Services (Trash, Streets, Sidewalks, Parks and Recreations, Water, Etc.)
- Billing
- Problems with my neighbors
So the next time you tag Liliana on a FB post in the neighborhood group or text her office about an issue you're looking at across the street, make sure you use this step first (instead).
A letter from District 5 regarding the launch of the District 5 Help Form:
"Hi friends,
I’m excited to share that our Office has launched a District 5 Help Form! This form will streamline the way our Office tracks and manages issues that are reported to us by our constituents. Whether it’s a pothole, missed garbage, or blighted property, it can be reported to us using this form!
Before filling out this form, folks should make sure they’ve submitted their issue to 311 and have their case number handy. This case number makes it easier for us to flag issues to City Departments and check their status. Once someone fills out our Help Form, our team is automatically notified, and we’ll get to work to see what we can do to resolve the issue. This form is the best way to contact our office regarding issues using city services, and starting today, folks will be asked to fill it out when sharing an issue with our team.
Can you all help us get the word out about this new form by sharing on your socials, website, and in your Facebook groups? The link to the form itself is here: https://form.jotform.com/ATLD5/district-5-help-form. This link will not change, so folks should bookmark it so that they always have easy access to this form! It can be filled out via phone too!
Thank you for your help in getting the word out! We’re excited about this new era of constituent services in District 5!
